James Reason’s work on Human Error Factors with understanding and accepting that humans will make mistakes, allows the Act Safety team to work through the Swiss Cheese Model to delve into the organization to identify the failing that lead to the occurrence. Their trainers from high risk steel and mining industries have vast experience in the use of ICAM (Incident Cause Analysis Method) as an investigation tool. Experienced in the role of investigation leader, ICAM facilitator and trainer, Act Safety’s aim is to seek collaboration from workforce and managers to develop a safer workplace.

Root Cause Analysis is shrouded in highbrow terms and generally regarded too complex to comprehend for the layman, let alone agree with any outcomes. Where the potential outcome means change; the opposition to change often resulted in the potential safety benefits lost. ICAM is a simple open and honest approach that properly used, removes the smoke and mirror syndrome, utilising engagement and buy in to investigation corrective actions is far more acceptable.

Using the ICAM model, participants will work individually and in teams investigating actual and scenario based incidents, developing causation charts, and corrective actions. The training is a simplified step by step process; there is no requirement for participants to have investigation experience. Expecting leaders and supervisors to know how to carry out effective investigations with no training is setting them up to fail.

Experience has shown this training works well with managers, supervisors and safety representatives from the same organization on the same course. Trainees receive a course workbook, investigation aids, and certificate of completion.  (Where required unit standards entered onto NZQA framework).  Management receives course competency register; hard and soft copy and trainees entered into database for refresher training. The course duration is two days.

ACT Safety is a Private Training Establishment and is authorized and accredited to enter unit standards onto the NZQA framework. After successfully completing their pre-moderated assessments either of the following units can be lodged for individuals (at no additional cost).

U.S. 17601 Produce an occupational health and safety incident investigation

 * U.S. 16686 Conduct an incident investigation at an extractive site.

 * Requires an actual additional investigation from the worksite.

ACT Safety offer country wide training and, as a PTE they offer unit standard training in Occupational Health & Safety and Fire and Emergency. They are also authorized and accredited to load trainees’ results onto the NZQA framework.
